"Walter Bass and Eleanor Teitsma continue as Executive Director and Treasurer, respectively." Also, Amagram from November 1979 has a list of all 1979 Amway Travel seminar qualifiers and Eleanor is listed as TEITSMA, INC. (George & Eleanore).

I don't understand, how the original list of AWA founding members in the movie could include an error. Maybe it was not a shot of the ORIGINAL document? And maybe it was Deb, who didn't know the correct spelling? Who knows?

This is strange. In a documentary "Profiles of the American Dream" there is a shot of the list of original members of The American Way Association and Eleanor is listed there as Eleanor Tietsma. Also, on the QBlog under the post named "Jody Victor and Deb" there is a discussion where Deb said: "I have a picture of the first ADA Board - and there were 3 women on it. One of them was a dear friend of ours named Eleanor Tietsma". Deb spelled Eleanor's surname as Tietsma TWO times - here and in another discussion.
